CARE MANAGEMENT to help seniors find in-home services and care providers to allow them to live independently for as long as possible, preventing premature nursing home placement
SENIOR CARE OPTION (SCO) will evaluate the long term care needs of persons over age 65 seeking Medicaid coverage for nursing facility care to determine if such care is needed or if other options are viable
HEALTH and nutrition programs include 20 congregate meal sites and home delivered meals through six sites; health and nutritional counseling may be provided to individual seniors, depending on available funding
LONG-TERM Care Ombudsman program provides certified volunteers, located in Buffalo and Phelps County long-term care facilities, to advocate for residents of the long-term care facilities
LEGAL SERVICES for persons age 60 and over and their spouses; specializes in Elderlaw and provides assistance with simple wills, powers of attorney for health care, living wills, durable powers of attorney, Medicare/Medicaid, etc.
MEDICAID WAIVER in-home services, including assisted living, for those age 65 and older with care needs; must meet income and resource guidelines as well as other eligibility criteria
INSURACE ASSISTANCE - SHIIP (Senior Health Insurance Information Program) assists seniors regarding health insurance issues such as Medicare, Medicaid, Medicare supplemental insurance, long term care insurance, group health insurance, etc.
FAMILY CAREGIVER PROGRAM provides caregiver training and education, information on available services, caregiver support, and funding for emergency response systems
RESPITE CARE grants to provide temporary relief for the permanent caregiver, up to $750 per year, per family; provided through the Family Caregiver Program
